Job Summary

We are seeking a skilled Internal Medicine Nurse Practitioner to provide comprehensive primary care specifically for patients receiving Medication-Assisted Treatment (MAT) for substance use disorders. The NP will play a crucial role in managing the overall health and wellness of MAT patients, ensuring holistic and coordinated care.

Key Responsibilities

Conduct comprehensive assessments of MAT patients, including medical history, physical examinations, and laboratory evaluations to identify health issues and monitor progress.
Manage and adjust MAT protocols in accordance with best practices and regulatory guidelines, ensuring optimal dosing and adherence.
Oversee and manage co-occurring chronic medical conditions (e.g., hypertension, diabetes) to promote overall health and well-being of patients.
Provide education on the effects of medications, lifestyle modifications, and strategies for managing substance use disorders and associated health issues.
Collaborate with a multidisciplinary team, including therapists and psychiatrists to create comprehensive care plans for patients.
Schedule and conduct regular follow-up appointments to monitor patient progress, adjust treatment plans as necessary, and provide ongoing support.
Maintain accurate and timely documentation of patient assessments, treatment plans, and progress notes in accordance with legal and ethical standards.
Advocate for patients' needs and assist them in accessing community resources and support services related to substance use recovery and overall health.

Requirements

Knowledge of professional nursing theory, standard nursing protocol and the legal implications of nursing practice, including statutes and regulations relevant to advanced practice nursing.
Ability to orally communicate effectively with others, with or without the use of an interpreter.
Ability to communicate effectively in writing using the English language, with or without the use of auxiliary aids or services.
Ability to review, understand, and apply concepts presented in training programs, conferences, and/or professional literature.
Clear understanding that FoundCare provides information on educating individuals about safer sex practices, which could include but not be limited to: exposure to explicit language, explicit printed material, and descriptions of explicit sexual activities, as part of the agency's mission in the prevention and transmission of HIV disease.
Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties.
Transport him/herself to other departments, conference rooms, and on occasion, to off-site locations to accomplish job responsibilities, attend meetings, workshops, seminars, etc.
Accomplish job duties using various types of equipment/supplies, e.g. pens, pencils, calculators, computer keyboard, telephone, etc.

Minimum Qualifications

Education: Graduate with an MSN or DNP in Family Practice.
Licensure: Current state licensure as a Nurse Practitioner.
Experience: Prior experience in internal medicine and familiarity with substance use disorders and MAT protocols.
Skills: Strong clinical assessment and diagnostic skills, excellent communication, and the ability to work effectively within a multidisciplinary team.
Certifications: Board Family Nurse Practitioner (FNP) preferred; Certification in Addiction Medicine (or related) is a plus.
