Job Description: We are seeking a Psychosocial Rehabilitation Specialist to join our team. The successful candidate will have a Master's degree in social work from an accredited university and at least 5 years of experience working as a medical social worker. The role involves providing clinical therapy services, coordinating SWING and Skilled Nursing Facility admissions, discharges, and Utilization Review, and participating in telehealth programs.
Responsibilities:
- Teach in-service staff trainings on Social Services related topics.
- Coordinate facility participation in telehealth programs.
- Assist in grant writing.
Requirements:
- Master's degree in social work from an accredited university.
- At least 5 years of experience working as a medical social worker.
- Valid license as a Clinical Social Worker in the State of California.
- BLS/CPR certification.
Estimated Salary: $95,000 - $120,000 per year, depending on experience.
