Key Responsibilities:- Conduct comprehensive assessments of members' health status, identifying gaps in care and potential health risks.
- Develop and implement personalized care plans to address members' needs and improve clinical outcomes.
- Collaborate with healthcare providers to ensure timely and adequate communication, documentation, and follow-up care.
- Provide education and support to members on managing chronic conditions, including information on recommended testing and disease-specific care.
- Maintain accurate and compliant medical records, adhering to CMS/DHHS, Inovalon policy, and client requirements.
- Active, unencumbered license to practice nursing in Family, Adult, Gerontology, or Emergency Medicine.
- Knowledge of CMS Regulations and NCQA HEDIS Guidelines.
- Familiarity with ICD-9, ICD-10, and CPT-4 coding practices.
- Home Health experience a plus.
- Bi-lingual or multi-lingual a plus.
