About the Role

We are seeking a highly skilled and experienced MDS Coordinator to join our team at Brookdale Senior Living. As a key member of our clinical staff, you will play a critical role in ensuring the highest quality of care for our residents.

Key Responsibilities
  • Conduct and coordinate the development and completion of resident assessments, in accordance with state, federal, and company guidelines.
  • Oversee the generation of Minimum Data Set (MDS) for each Medicare patient and electronic transmission of required data within the mandated timeframe.
  • Coordinate the development and completion of the resident assessment (MDS) in accordance with current regulations and guidelines, including the implementation of CAAs and Triggers.
  • Evaluate each resident's condition and pertinent medical data, and develop and implement procedures with the Director, Clinical Services for the arrival of newly admitted residents.
  • Ensure that all assessments are completed and transmitted in a timely manner, and assist community directors and supervisors in scheduling resident assessment/care plan meetings.
  • Contact and assist in scheduling participation by outside members of the care plan team, including the resident's representative and/or other interested family members.
  • Develop and implement a written plan of care (preliminary and comprehensive) for each resident, identifying problems/needs, the amount of care, goals to be accomplished, and which professional service is responsible for care.
  • Ensure that the care plan includes measurable objectives and timetables to meet the resident's needs, as identified in the resident's assessment.
  • Ensure the generation and transmission of MDS is complete and timely, and maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
  • Develop, implement, and maintain an ongoing quality assurance program for the resident assessment/care plans, and monitor the community's QI and QM reports to ensure that appropriate corrective action can be implemented when potential problems occur.
  • Assist the resident and Discharge Planning Coordinator in completing the care plan portion of the resident's discharge plan, and participate in functions involving discharge plans as necessary.
  • Participate in community surveys (inspections) made by authorized government agencies, and provide leadership and participate in various committees, including the Interdisciplinary Care Plan Team, Policy Advisory, and Quality Assessment and Assurance.
  • Develop and participate in the planning, conducting, and scheduling of timely in-service training classes that include assessment skills or techniques needed to complete the assessment and MDS functions of the community.
Requirements
  • A Nursing Degree from an accredited college or university.
  • A minimum of two (2) years of experience as a supervisor in a hospital, nursing care community, or other related healthcare facility.
  • A minimum of six (6) months training experience in rehabilitative and restorative nursing practices.
  • Knowledgeable of general, rehabilitative, and restorative nursing and medical practices, procedures, regulations, and guidelines governing long-term care.
  • A current State LPN/LVN license.
Working Conditions

This role requires interaction with co-workers, residents, or vendors, and may involve exposure to communicable diseases and infections. The ability to lift up to 50 pounds is also required.
