As a Licensed Practical Nurse, you will be responsible for delivering patient-centered care, maintaining a safe and clean environment, and collaborating with the healthcare team to achieve excellent outcomes.
Key Responsibilities:
- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Maintain accurate and up-to-date patient records, including medical history, medications, and treatment plans.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions.
Requirements:
- Current Licensed Practical Nursing license in the state of Virginia.
- CPR certification required upon hire or willing to obtain.
