Weekend Registered Nurse RN Home Health Specialist

Key Responsibilities:- Assess patient needs and develop individualized care plans to ensure optimal health outcomes
- Provide high-quality nursing care to patients in their homes, including wound care, medication management, and education
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
- Document patient progress and outcomes to inform care planning and quality improvement initiatives
- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Associates Degree or Bachelor's Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with HomeCare HomeBase is preferred
