BAYADA Home Health Care is seeking an experienced Speech Language Pathologist to join our team in Millville, NJ.
This is a full-time opportunity to work with a multidisciplinary healthcare team, providing rehabilitative care and skilled nursing to clients in their homes.
Responsibilities:- Conduct thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
- Develop and implement treatment plans based on evaluations, physician's orders, and recommended goals.
- Continuously assess and revise the speech therapy care plan, participating in multidisciplinary care planning as needed.
- Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations related to patient care management and services provided.
- Current license as a Speech Language Pathologist in NJ.
- Certificate of clinical competence in speech from the American Speech and Hearing Association, or equivalent educational requirements and work experience.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
