Position Overview
The Home Health Registered Nurse (RN) plays a crucial role in delivering and documenting skilled nursing services in alignment with the established care plan and medical directives for each patient, while consistently maintaining confidentiality and professional boundaries.
Schedule: PRN visits are required Monday through Friday, with an availability of 10-15 hours or visits.
Essential Responsibilities:
- Formulate a comprehensive care plan based on physician orders, client feedback, and nursing evaluations, incorporating medical interventions along with measurable objectives and outcomes.
- Provide education to clients and their families tailored to the specific needs of the client.
- Effectively orient and train primary caregivers to ensure optimal care and functioning for each client.
- Facilitate a seamless continuum of care through meticulous documentation and prompt communication with the client's physician and other healthcare providers.
- Deliver care in the patient's residence utilizing a range of skills, including phone triage, patient education, assessment, wound care, infusions, catheter management, and PICC line dressing changes.

Qualifications:
- Must possess an active RN License in the relevant state.
- Current CPR certification is required.
Preferred Qualifications:
- Experience in Medicare Skilled Nursing.
- Basic knowledge of Oasis documentation.
- At least one year of RN experience in a healthcare environment.
