Job Title: Direct Service Provider

Job Summary:

The Direct Service Provider plays a vital role in supporting individuals with mental health and substance abuse needs in community, home, residential, or other treatment settings. This position requires a compassionate and skilled individual to provide daily oversight and support to consumers, promoting their independence, self-care, and overall well-being.

Key Responsibilities:

  • Assist consumers in self-care, self-direction, and independence, as directed by their service/treatment plan.
  • Perform light household cleaning, plan shopping lists, and assist with meal preparation and grocery shopping when assigned.
  • Organize and participate in leisure activities with consumers, providing instruction and guidance on appropriate behavior and interactions.
  • Accompany consumers in the community for activities, including medical appointments, grocery shopping, and treatment plan meetings.
  • Monitor consumers in self-administration of medications or administer medications if certified.
  • Provide education and supportive services to consumers.
  • Document consumer observations and behaviors according to policy.
  • Monitor consumer behavior and address inappropriate behaviors according to the consumer's treatment plan or consultation with program leadership.
  • Oversee consumer safety and direct emergencies that may arise during shift.
  • Utilize effective communication skills and verbal de-escalation techniques.
  • Assist in behavioral management or crisis intervention as indicated by training and consumer's service/treatment plan.
  • Transport consumers as needed, using Prestera Health Service's vehicle or personal vehicle.
  • Maintain a working knowledge of Prestera policies, procedures, and licensure regulations governing service delivery.
  • Adhere to confidentiality, HIPAA, and risk management policies and procedures.
  • Utilize technology to facilitate services and perform job efficiently and effectively.
  • Meet training requirements, including CPR-First Aid and crisis intervention.

Qualifications:

  • Valid driver's license required.
  • Experience working with mental health and substance abuse populations preferred.
  • Knowledge of community resources preferred.
  • Ability to learn and navigate the EHR system effectively.
  • Excellent interpersonal skills and ability to develop trusting relationships.
  • Good written and verbal communication skills, along with good time management and organizational skills.

Education and Training Requirements:

  • High School Diploma or General Educational Development Certificate required.
  • Maintain First Aid/CPR certification.
  • Maintain current crisis prevention/intervention training.
