Job Summary

We are seeking a highly organized and detail-oriented Patient Care Coordinator to join our team at Amedisys. As a key member of our healthcare operations team, you will be responsible for scheduling patient visits, coordinating care, and ensuring the efficient utilization of clinical resources.

Key Responsibilities
  • Scheduling and Coordination: Schedule patient visits, including Oasis visits, and ensure timely completion of assessments.
  • Clinician Scheduling: Prepare weekly clinician schedules, review daily or as needed, and ensure productivity standards are met.
  • Geographic Optimization: Assign and schedule clinicians in the most efficient geography, maximizing clinician efficiency and productivity.
  • Missed Visits: Ensure missed visits are scheduled by the end of the current week or referred to the Clinical Manager for resolution.
  • Communication: Manage calls from patients and field staff related to scheduling issues, and refer clinical and performance-related issues to the Clinical Manager.
  • Hospitalized Patients: Monitor hospitalized patients, communicate with the team, and ensure follow-up as necessary.
  • Schedule Management: Complete requested schedules, ensuring all orders and services are included as required.
  • Collaboration: Work collaboratively with field clinicians, patients, and other office personnel to ensure timely communication and efficient care delivery.
  • Reporting and Workflow: Run applicable reports, respond to workflow, and take appropriate actions.
  • Supply Management: Maintain supply closets, ensure supplies are within expiration dates, and serve as liaison with the field for patient supply needs.
  • Referral and Intake: Assist with internal or external transfer of patients between care centers and/or Hospice services, and may be responsible for the referral and intake process in the absence of an Intake Coordinator.
Qualifications
  • Licensure: Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse specific to the state the employee is assigned to work by the company.
  • Education: High School diploma or GED equivalent.
  • Experience: One (1+) year of administrative experience in a healthcare environment.
