As a Clinical Wound Care Specialist at Kaiser Permanente, you will play a vital role in providing comprehensive care to patients with wound, ostomy, and continence needs. Your expertise will be essential in managing patient outcomes and coordinating clinical aspects of care across the continuum of care.

Key Responsibilities:

  • Develop and implement individualized care plans to address wound, ostomy, and continence needs.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and patient education.
  • Monitor patient progress and adjust care plans as needed to achieve optimal outcomes.

Requirements:

  • Graduation from an accredited Wound, Ostomy, and Continence Nursing Education Program required.
  • Registered Nurse License (Virginia, Maryland, or Compact License) within 6 months of hire.
  • Basic Life Support certification.
  • Wound Ostomy Continence Nurse Certificate within 12 months of hire.

Preferred Qualifications:

  • Bachelor of Science in nursing preferred.
