Job Summary: Centra Healthcare Solutions is seeking an experienced Speech Language Pathologist (SLP) to work in the specialty area of Skilled Nursing Facilities (SNF) and Long-Term Care (LTC).

About the Role: As a Speech Language Pathologist, you will be responsible for assessing and treating patients with communication and swallowing disorders. You will work closely with the healthcare team to develop and implement individualized treatment plans to improve patient outcomes.

Responsibilities:

  • Conduct comprehensive assessments of patients' communication and swallowing abilities
  • Develop and implement individualized treatment plans to address patients' communication and swallowing needs
  • Provide therapy sessions to patients, including speech, language, and swallowing therapy
  • Collaborate with the healthcare team to ensure comprehensive care and coordination of services
  • Document patient progress and treatment plans in accordance with regulatory requirements

Requirements:

  • Master's degree in Speech-Language Pathology from an accredited program
  • Current licensure as a Speech Language Pathologist in the state of practice
  • 1 year of recent experience as a Speech Language Pathologist in a SNF/LTC setting
