As a Case Manager RN, you'll play a critical role in coordinating patient services through an interdisciplinary process, providing a clinical and psychosocial approach throughout the continuum of care.

Key Responsibilities:
  • Assess patients to determine appropriateness of admission, continued hospitalization, and appropriate level of care
  • Facilitate timely care delivery at the right time, in the right setting, by following CMS guidelines and collaborating with stakeholders
  • Develop and implement discharge plans in partnership with the clinical team, patient, and/or patient's representative
  • Monitor and report quality and risk management issues as appropriate
Requirements:
  • Associate's Degree (or higher) in Nursing
  • Current, unrestricted RN license in the state of employment
  • 1+ years of experience with/knowledge of community resources, policies, and procedures
  • 1+ years of clinical experience with/knowledge of age groups, including but not limited to the unborn child through geriatric age group
Preferred Qualifications:
  • Bachelor of Science
  • 2+ years of case management experience
  • Case Management Certification
  • Strong analytical, critical thinking, and organizational skills
