Key Responsibilities:

  • Evaluate member needs, resources, and develop plans for optimal outcomes.
  • Assist with creating ongoing long-term care plans/service plans and identify necessary providers.
  • Cookdiniate between members, families/caregivers, and care provider teams to ensure accessible services.

Requirements:

  • Bachelor's degree and 2-4 years of related experience or equivalent experience acquired through accomplishments.
  • Strong skills in case management, social work, and human services.
  • Licensed Social Worker or LPN/LVN license is strongly preferred.
