Job Summary

The Physical Therapist will be responsible for providing high-quality physical therapy services to patients in their own homes, assessing patients' conditions, developing personalized treatment plans, and delivering therapy to improve mobility, reduce pain, and restore function.

Key Responsibilities
  • Assess patients' conditions and develop personalized treatment plans
  • Deliver physical therapy services to patients in their homes
  • Travel to patients' homes and adapt treatment plans to suit home environments
  • Ensure high-quality care and patient safety
  • Assist in development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy
  • Treat clients to relieve pain, develop or restore functional and maintain maximum performance
  • Teach, supervise, and counsel the family and client in the physical therapy program
  • Instruct client in care and use of adaptive devices
  • Record evaluation data, treatments, and client's response to therapy intervention
  • Participate in case conferences, physical therapy staff meetings, and in-service programs
  • Supervise the home health aide every 14 days when physical therapy is the only professional discipline involved in the client's care
Requirements
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A.
  • At least one year of supervisory experience in home care
  • Must be compliant with conditions of participation for Center for Medicare/Medicaid Services
