Job Description

The Care Manager II (RN/LICSW) plays a critical role in our care coordination program, ensuring that members receive coordinated and effective care. Key responsibilities include:

Key Responsibilities:
  1. Engage members with complex medical needs in achieving their optimal level of health through self-management.
  2. Conduct comprehensive assessments to identify problems/barriers for care coordination and develop appropriate care management interventions.
  3. Cordinate physical, behavioral health, and social services to support member goals.
Requirements

To succeed in this role, you must have:

  • Associate's Degree in Nursing or Bachelor's Degree in Social Work (LICSW).
  • Minimum 3 years of clinical practice experience as a RN or LICSW working with adults and/or pediatrics with complex medical and behavioral health diagnoses within an acute care, homecare, or community health setting.
  • Minimum 1 year of case management experience supporting Medicaid members within a managed care organization.
Compensation and Benefits

We offer a competitive salary range of $80,000 - $95,000 per year, depending on experience. Our comprehensive benefits package includes:
