Job DescriptionThe Care Manager II (RN/LICSW) plays a critical role in our care coordination program, ensuring that members receive coordinated and effective care. Key responsibilities include:
Key Responsibilities:- Engage members with complex medical needs in achieving their optimal level of health through self-management.
- Conduct comprehensive assessments to identify problems/barriers for care coordination and develop appropriate care management interventions.
- Cordinate physical, behavioral health, and social services to support member goals.
To succeed in this role, you must have:
- Associate's Degree in Nursing or Bachelor's Degree in Social Work (LICSW).
- Minimum 3 years of clinical practice experience as a RN or LICSW working with adults and/or pediatrics with complex medical and behavioral health diagnoses within an acute care, homecare, or community health setting.
- Minimum 1 year of case management experience supporting Medicaid members within a managed care organization.
We offer a competitive salary range of $80,000 - $95,000 per year, depending on experience. Our comprehensive benefits package includes:
