Job DescriptionWe are seeking a highly skilled Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a vital role in ensuring the smooth delivery of patient care services.
Responsibilities:- Assist patients with daily living activities and provide emotional support as needed
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Maintain accurate and detailed records of patient interactions and progress
- Communicate effectively with patients, families, and healthcare professionals
- Participate in quality improvement initiatives to enhance patient care and services
To be successful in this role, you will need:
- A minimum of 1 year of experience in a healthcare setting
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Strong problem-solving and critical thinking skills
- Basic computer skills and proficiency in electronic medical records
We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, plus benefits including medical, dental, vision, and a generous paid time off package.
