I. POSITION OBJECTIVE

II. ESSENTIAL FUNCTIONS

  • Acts as the lead nurse for the assigned care area, ensuring continuous clinical oversight.
  • Gathers information on residents and accurately records their conditions.

Documentation includes:
Wound care, medication administration records, monthly summaries, chart audits, and order confirmations. Monitors the electronic medical record dashboard and addresses identified issues promptly.

  • Conducts clinical assessments such as blood glucose and oxygen saturation as required. Administers prescribed medications and treatments, including IV fluid monitoring.
  • Prepares for and conducts weekly physician rounds as applicable. Accepts physician orders, including verbal and telephone orders, in accordance with regulations and nursing standards.
  • Assists in compiling the 24-Hour Report and Care Roster.
  • Guides staff in resident care and communicates changes in residents' conditions to families, physicians, and relevant departments.
  • Maintains CPR certification and initiates CPR as per facility protocols.
  • Provides direct resident care, including assistance with bathing, feeding, bed-making, and toileting.

III. OTHER JOB FUNCTIONS
Performs specific tasks and responsibilities as assigned by the supervisor.

IV. KNOWLEDGE / EXPERIENCE REQUIREMENTS
  • Graduation from an accredited nursing program as a Licensed Practical Nurse.
  • Basic computer skills are required.
  • Must be comfortable working with patients who have infectious diseases.
  • Must be fully vaccinated against COVID-19 and influenza in accordance with health guidelines.

V. LICENSES/ CERTIFICATIONS
  • Current LPN licensure from the state nursing board.
  • CPR certification is mandatory.
