Position Overview

As a member of the Facility Resource Pool, you will provide clinical leadership for all facets of patient care within an acute inpatient environment, emphasizing patient and family-centered care. You will demonstrate clinical proficiency by employing evidence-based practices and research throughout the nursing process: assessment, planning, implementation, evaluation, and outcomes. Your role will involve integrating knowledge of safety protocols, cost management, and quality enhancement into the nursing workflow. You will identify priorities for patients, units, and the organization, incorporating these into your nursing practice.

Compliance with all organizational policies and procedures is essential, including but not limited to: licensure renewal, mandatory training, employee health evaluations, attendance policies, dress code adherence, patient confidentiality, infection control, and medication administration. You will maintain an understanding of regulatory agency requirements (TJC, DHEC, CMS) and provide care within the scope of the South Carolina Nurse Practice Act.

This role requires flexibility to float between various nursing units within the assigned facility.

Staff may be scheduled for weekend, holiday, and on-call shifts in the acute inpatient setting.Location Details

This position is based at Hillcrest.Travel to other campuses may be required as necessary.

Key Responsibilities

Conducts thorough data collection and nursing assessments utilizing appropriate standards and tools specific to the area and/or age group. Completes necessary documentation. Recognizes subtle or significant changes in patient status and responses to interventions. Establishes nursing priorities for patient care plans based on assessments and relevant data, validating these priorities with the healthcare team and documenting them appropriately.

Care Planning and Outcomes:

Creates individualized care plans based on the patient's age and assessed needs, collaborating with the patient, family, physician, and interdisciplinary healthcare team. Updates care plans in response to changes in patient conditions and communicates these updates to team members.

Identifies realistic and measurable goals and outcomes to be achieved prior to discharge, validating these with the patient and/or family.

Demonstrates respect for the cultural, spiritual, and belief systems of patients and families during care planning.

Includes discharge planning to ensure continuity of care and a smooth transition after discharge, including appropriate referrals.

Implementation:

Delivers direct care to assigned patients in accordance with the established nursing plan.

Performs specific tasks based on organizational, departmental, and unit policies and procedures within the RN scope of practice.

Delegates nursing tasks appropriately to healthcare team members, providing written assignments as necessary and ensuring task completion.

Continuously monitors the quality of care provided, ensuring proper surveillance of assigned patients.

Coordinates and organizes tasks based on patient and unit priorities, utilizing internal and external resources effectively. Incorporates cost containment principles into nursing care.

Takes responsibility for patient safety by maintaining a secure environment, adhering to safety procedures, protocols, and patient care standards.

Utilizes relevant technology in daily nursing practice.

Employs performance indicators to gather data, monitor quality, and assess effectiveness while delivering nursing care. Completes assigned performance improvement monitoring tasks.

Evaluation:

Evaluates the effectiveness of the care plan based on patient responses to interventions, documenting progress toward expected outcomes and revising the care plan as necessary. Reports changes in patient conditions and refers unresolved issues to appropriate healthcare team members. Engages in discussions about outcomes with peers and other team members. Identifies opportunities for quality improvement and incorporates performance improvement recommendations into the care plan as appropriate.

Patient and Family Education:

Assesses the learning needs of patients and families and provides education accordingly by:

- Using familiar language and terminology

- Providing examples to clarify concepts

- Implementing and evaluating teaching plans that encourage patient/family involvement

- Utilizing resources such as Micromedex and computer tools

Communicates the educational needs of patients and families to others in the healthcare continuum.

Professional Accountability:

Reflects on nursing practice in relation to professional standards, state regulations, policies, and current knowledge. Takes personal responsibility for maintaining competency through ongoing education and professional activities to enhance community healthcare. Maintains professional licensure and completes mandatory education in a timely manner. Identifies strengths and areas for improvement in practice.

Ensures that documentation and communication focus on relevant information to meet patient and family care needs.

Acts as the patient's advocate and care coordinator, collaborating with physicians and other healthcare team members.

Supervisory Responsibilities

This is a non-management position reporting to a supervisor, manager, director, or executive.

Minimum Qualifications

Associate's Degree in Nursing; BSN preferred.

Two years of experience as a registered nurse.

Licensure/Certifications Required

Licensed to practice as a Registered Nurse in South Carolina.
