Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Managed Care Case Manager. This is an excellent opportunity for a dedicated professional to make a meaningful impact in the lives of our patients.

About the Role

This position coordinates care and services for defined patient populations across the acute care continuum, ensuring seamless transitions and optimal outcomes. As a key member of our interdisciplinary team, you will work closely with healthcare providers, family members, and other stakeholders to develop and implement personalized care plans.

Responsibilities
  • Conduct thorough assessments of patient needs and develop targeted care plans to address medical, emotional, and social requirements.
  • Collaborate with healthcare teams to ensure effective communication, coordination, and resource allocation.
  • Maintain accurate records and reports, utilizing clinical data to inform decision-making and drive quality improvement initiatives.
  • Stay up-to-date with relevant clinical protocols, standards, and regulatory requirements, applying this knowledge to enhance patient care and safety.
Requirements

To be successful in this role, you should possess:

  • A degree from an accredited baccalaureate nursing program (Bachelor's degree in Nursing).
  • Current American Heart Association (AHA) Healthcare Provider CPR card.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders.
  • Critical thinking, problem-solving, and organizational skills, with the capacity to manage multiple priorities and adapt to changing circumstances.
  • A willingness to learn and grow, with a commitment to ongoing professional development and self-improvement.
Work Environment

This is a full-time day shift position, reporting to the Director of CVHP Operations. You will work in a dynamic and supportive environment, collaborating with a multidisciplinary team to deliver high-quality patient care.

Estimated Salary Range

$85,000 - $110,000 per year, depending on experience and qualifications.
