Role Overview:We are seeking a dedicated and high-performing Practical Nursing Specialist to join our Clinical Care Team, collaborating with Nurse Practitioners to manage the health of a defined patient population.
This role involves traveling to patients' homes, fostering meaningful relationships while delivering care. Candidates should be adept with mobile technology and possess a strong commitment to exceptional patient service.
A typical day may encompass:
- Conducting comprehensive clinical assessments in patients' homes
- Performing telephonic evaluations
- Providing patient education and support
- Ensuring adherence to treatment plans
- Connecting patients with community resources
- Conducting routine diagnostic tests
- Documenting assessments accurately in our systems
This position offers both personal and professional fulfillment, allowing the successful candidate to provide healthcare services aligned with their training. Workdays will blend remote office work with community visits, offering flexibility while maintaining a focus on patient-centered care.
Key Responsibilities:- Conduct thorough patient assessments, including vital signs, and monitor changes in health status
- Collaborate with Nurse Practitioners to implement and adjust care plans
- Educate patients on treatment goals and address barriers to care
- Evaluate treatment effectiveness and recommend necessary modifications
- Communicate effectively with healthcare teams, including physicians, nurses, and social workers
- Document patient care and services promptly and accurately
- Maintain hygiene and safety standards consistently
- Foster a compassionate environment for patients and their families
As part of our market team, you may be required to cover patients within a designated area, typically within a 60-mile radius of your home. Our scheduling procedures are designed to optimize patient care while minimizing travel time.
Qualifications:- Completion of an accredited LPN program
- Minimum of 2 years of experience as an LPN
- Strong knowledge of nursing care practices and emergency procedures
- Valid and active nursing license in the state
- Reliable transportation and current CPR certification
- Experience in cardiac or pulmonology care preferred
- Bilingual skills (English/Spanish) are a plus
- Effective verbal and written communication skills
- Able to assess and respond to patient needs quickly
- Comfortable working with patients with varying health literacy levels
- Ability to prioritize tasks and manage time efficiently
- Proficient with electronic medical records and mobile applications
This role operates in a remote professional setting, utilizing standard office equipment. The physical demands include the ability to lift up to 20 pounds and perform essential functions effectively.
