Position Summary

LEAN ON ME HOME CARE INC is seeking a dedicated Personal Care Assistant to become a vital part of our team. In this role, you will provide individualized support to clients in their residences, adhering to personalized care plans. Your compassionate approach will not only ensure quality care but also foster meaningful connections with those you assist. We value attention to detail, organizational skills, and a commitment to maintaining a nurturing environment that addresses the needs of our clients. Excellent communication abilities, both verbal and written, are essential for success in this position. Previous experience in home health or caregiving is advantageous.

Key Responsibilities
  • Support clients with everyday tasks, including preparing pre-cooked meals, engaging in light activities like puzzles, and maintaining social interaction.
  • Assist clients with household duties such as changing bed linens, laundry, cleaning, and personal care assistance.
  • Document patient care, observations, and any changes in condition to discuss with supervisory staff.
  • Foster relationships with clients through conversation and reading to promote mental well-being.
  • Encourage clients to participate in physical exercises or other stimulating activities.
Required Qualifications
  • Completion of an accredited Personal Care Assistant training program.
  • High School Diploma or equivalent.
  • Minimum of one year of relevant professional experience.
  • Valid driver's license.
  • CPR certification is preferred.
