Position Overview:
The Patient Education Specialist collaborates with patients referred for assistance by providing education and coordinating services for chronic disease self-management. The primary goal of the Patient Education Specialist is to enhance health outcomes through care coordination, patient education, fostering trust between patients and healthcare providers, and improving communication and continuity of care. The Patient Education Specialist will work alongside other healthcare team members to ensure effective patient education, preventive care, and disease management.
Main Responsibilities:
- Serves as an advocate for individual care needs by identifying and communicating opportunities for care interventions, addressing gaps in care as necessary.
- Coordinates care services for patients with chronic conditions such as diabetes, hypertension, and asthma, adhering to established clinic policies and procedures, objectives, quality improvement initiatives, safety, and infection control standards.
- Implements targeted strategies to enhance health, functional, or quality of life outcomes, including disease management and patient education plans.
- Monitors hospitalizations, emergency visits, and institutional admissions to identify issues and implement follow-up for improved outcomes.
Qualifications:
Education:
- High school diploma or equivalent required; completion of an accredited Licensed Practical Nursing program or RN program; Active Florida LPN or RN license.
Experience:
- At least two (2) years of experience as a Licensed LPN or RN in Health Care or Managed Care is preferred.
- Practical experience in a patient educator role or documented training in patient education.
- Demonstrated ability to work effectively as part of an interdisciplinary team and proven experience in verbal and written communication and case management services.
Special Skills:
- Understanding of health implications related to chronic diseases and associated risks.
- Exceptional oral and written communication skills.
- High level of initiative and leadership capabilities.
- Strong advocacy skills.
- Proven ability to manage multiple projects and prioritize effectively, along with excellent time management skills.
Additional Requirements:
- Bilingual: English/Spanish is advantageous.
- Able to work flexible hours and at various CHC locations as required.
