With a focus on prevention and wellness, you'll work closely with patients to develop personalized care plans that address their unique needs.
Key Responsibilities
* Provide routine wellness care, chronic disease management, and urgent visits to patients
* Develop and implement personalized care plans that address patient needs
* Collaborate with patients to set and achieve health goals
* Stay up-to-date with the latest medical research and best practices through ongoing education and training
