Job Title: Physical Therapist Home Health

Altru Health is seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will work with patients in their homes to evaluate and treat physical disabilities caused by injury, disease, or surgery.

Key Responsibilities:
  • Evaluate patients and develop treatment plans to improve mobility and ease pain.
  • Work with interdisciplinary teams to coordinate care and ensure patient progress.
  • Monitor and document patient progress, making adjustments to treatment plans as needed.
  • Provide patient education and support to promote independence and self-care.
  • Supervise physical therapist assistants and therapy aides to ensure high-quality care.
Requirements:
  • Physical Therapist licensure in North Dakota and Minnesota.
  • Basic Life Support (BLS) certification within 90 days of hire.
  • Valid driver's license with an acceptable driving record.
Working Conditions:

This role requires frequent driving and working in patients' homes. You must be able to lift up to 50 pounds and stand for long periods.
