Job Summary:

Ovation Primary Care & Hospice is seeking a compassionate and skilled Hospice Certified Nursing Assistant to join our team. As a Hospice CNA, you will play a vital role in providing high-quality patient care and support to our patients and their families.

Key Responsibilities:

  • Provide personal care to patients under the direction of a registered nurse, including bathing, dressing, and grooming.
  • Assist patients with ambulation, transfer, and range of motion as documented on the Hospice Aide Plan of Care.
  • Document care provided according to organization policy and report any deviations from the Plan of Care to the Case Manager.
  • Use Durable Medical Equipment safely and take vital signs when on Hospice Aide Plan of Care.
  • Foster a cooperative effort among personnel by understanding the functions of other persons involved in patient care and participating in team and staff conferences.
  • Provide respite for patients' families/caregivers when on-site, as appropriate.
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities and Interdisciplinary Group (IDG) meetings, as appropriate.

Requirements:

  • Meet training requirements by the state of Arizona.
  • One year experience as a CNA, hospice aide, or home health aide.
  • At least 18 years old.
  • Must be a licensed driver with an insured automobile in good working order.
  • The ability to communicate well, both verbally and in writing.
  • Ability to work as a team and also independently.
