Wound Care Nurse Job Description

We are seeking a skilled Wound Care Nurse to join our team at Huntington Hills Center for Health & Rehabilitation. As a Wound Care Nurse, you will be responsible for assessing, treating, and monitoring wound conditions in collaboration with the interdisciplinary team.

Key Responsibilities:
  • Provide oversight and expertise of wound care and prevention program for residents and staff
  • Conduct comprehensive wound assessments and develop individualized care plans for patients exhibiting skin complications
  • Provide direct wound care interventions, including dressing changes, debridement, and wound irrigation
  • Review, update and direct the implementation of wound care & prevention policies and procedures
  • Monitor wound healing progress and evaluate the effectiveness of treatment modalities
  • Educate residents, families, and staff on wound care protocols and prevention strategies
  • Collaborate with physicians, wound care specialists, and other healthcare professionals to coordinate care
  • Maintain accurate documentation of wound assessments, interventions, and outcomes for regulatory compliance and reimbursement

Requirements:
  • RN license in the state of New York for the full-time role and an RN or LPN license in the state of New York for the weekend role
  • Degree in nursing, epidemiology, biology, microbiology preferred
  • 2+ years in wound care management, preferably in a Long-Term Care setting
  • Familiar with research-backed wound care and cleansing products
  • Certified Wound Care Nurse (CWCN) Certification preferred
  • Strong clinical assessment, organizational, and critical thinking skills
  • Compassion and empathy in caring for residents with complex wound care needs
  • Interest in the nursing needs of the aged and the chronically ill with the ability to work with both
  • Excellent communication and interpersonal abilities
  • Ability to work effectively in a multidisciplinary team environment
