Position Details:This role typically operates Monday through Friday during daytime hours, with no weekend or holiday shifts. After a period of integration with the team, there may be an opportunity for remote work one day per week.
Key Responsibilities:As an Outpatient Clinical Pharmacist, your responsibilities will include:
- Implementing pharmacy services and clinical programs in alignment with established policies and regulations.
- Conducting thorough reviews of patient records to assess the suitability of medication therapies and identify patients for outreach initiatives.
Evaluating patient conditions and addressing any obstacles to medication adherence to ensure comprehensive treatment of all issues.
- Identifying unaddressed health concerns and referring patients to the appropriate healthcare providers.
Formulating effective medication strategies that minimize the potential for adverse effects.
- Providing medical personnel with essential medication information regarding both new and existing therapies to support safe prescribing practices.
Assessing the outcomes of pharmaceutical interventions and collaborating with healthcare professionals to ensure optimal patient care.
- Advising on the proper procurement, storage, and administration of medications within the clinical environment.
- Prescribing and discontinuing medications, adjusting dosages, and ordering laboratory tests in accordance with collaborative practice agreements authorized by the patient's physician.
- Maintaining precise documentation of medication plans and patient progress to meet the requirements for effective patient care and support billing practices.
- Engaging in continuous quality improvement efforts by participating in data collection and analysis, and developing program modifications based on this analysis.
PharmD Degree with a minimum of two (2) years of experience in a retail or outpatient pharmacy setting, or completion of a pharmacy residency program.
Preferred qualifications include completion of a PGY2 Ambulatory Pharmacy Residency or experience as an Outpatient Clinical Pharmacist, along with board certification in a relevant area (e.g., BCACP, BCPS).
Physical Requirements:Ability to perform tasks involving hearing, lifting, manual dexterity, pulling/pushing, vision, sitting, speaking, squatting/kneeling, standing, and walking.
