Job SummaryThe Social Worker will work under the supervision of the Home Health and Hospice Director to evaluate and provide bereavement support and needed services for Home Health, Hospice, and Bereavement clients. This role requires strong communication and delegation skills, as well as the ability to work effectively with patients, families, and staff.
Essential Functions- Establish and provide bereavement support, including individual support sessions and bereavement support groups for patients/families and the community.
- Organize and assist with community bereavement events.
- Demonstrate teamwork through communication and delegation of patient needs.
- Consult and provide social services for Home Health and Hospice patients, actively participating on the Hospice IDT team.
- Consult patients and families on services provided by Hospice and Home Health.
- Perform initial social service evaluation and psychosocial and spiritual assessment of patient.
- Implement a social service plan of care for Hospice and/or Home Health patients and update plan of care as indicated by patient and family needs.
- Facilitate assistance and coordinate services for patients and families, including use of community resources, to assist with financial needs, Advance Directives, funeral planning, etc.
- Facilitate use of ancillary disciplines appropriate to the patient's plan of care.
- Document interventions and interactions with patients and families in the medical record.
- Comply with the hospital's corporate compliance program, including the Code of Conduct, laws and regulations, and hospital policies and procedures.
Education: Bachelor's degree in social work from an institution accredited by the Council on Social Work Education, or a bachelor's degree in psychology, sociology, or other field related to social work.
Certifications: See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
Experience: Preferred experience in social work, with knowledge of bereavement support and community resources.
Knowledge, Skills, and AbilitiesThe ideal candidate will have strong analytical and reasoning skills, with the ability to define problems, collect data, establish facts, and draw conclusions. They will also have excellent verbal communication skills, with the ability to mediate and interact with people from diverse populations.
The ability to maintain continuing education requirements and all certifications and licenses as required is essential. The candidate should also have knowledge of developmental stages and be able to apply developmental theories/concepts when planning and implementing care for the appropriate age of the patient.
The ability to demonstrate knowledge of and maintain current knowledge and skill in the following age groups is also required: Infant (Neonate to Age 1), Pediatric (Age 1-11), Adolescent, Adult, and Geriatric (Age 65 and older).
