We are seeking a skilled Case Management RN to join our team at TravelNurseSource. As a key member of our healthcare team, you will be responsible for developing, implementing, and evaluating individual patient care plans.
Key Responsibilities:- Manage patient care plans to ensure optimal outcomes
- Collaborate with patients, families, and healthcare providers to coordinate care
- Develop and implement individualized care plans to meet patient needs
- Monitor patient progress and adjust care plans as needed
- 1-2 years of experience in case management or a related field
- Active license or accreditation in one of the 50 US states
- Additional requirements and certifications as requested by the facility
