We are seeking a skilled House Calls Nurse Practitioner to join our team, providing personalized medical care to seniors and patients in home and community settings.

The ideal candidate will have a strong knowledge base of evidence-based medical practices, excellent interpersonal communication skills, and the ability to work independently with minimal supervision.

Key Responsibilities:

• Performing comprehensive medical assessments and treating illnesses in home and community settings
• Collaborating with healthcare professionals to ensure interdisciplinary care and maintaining accurate patient records
• Providing referrals to other healthcare practitioners as necessary

Requirements:

• Active state Nurse Practitioners license
• Current Nurse Practitioner National Board Certification
• Master's degree in Nursing (MSN)
• Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
