Job Description

As a Physical Therapist II in our Home Care Services team, you will play a vital role in delivering exceptional patient care and support in the comfort of their own homes. Our team is dedicated to excellence in clinical practice, education, and interdisciplinary collaboration.

Key Responsibilities:
  • Develop and implement individualized treatment plans that address patient deficits and areas of concern, ensuring a safe and goal-focused approach.
  • Evaluate patient responses and modify treatment plans according to Vanderbilt Health's and VUMC's standards.
  • Administer and score standardized tests, interpreting results to inform patient care and communicate effectively with the healthcare team.
  • Participate in quality improvement activities and identify opportunities for cost-effective measures in clinical care.
  • Manage clinical assignments efficiently, utilizing human and other resources to ensure high-quality patient care.
  • Provide therapeutic interventions, evaluate patient responses, and modify treatment plans as needed.
  • Assure coordination of care for all patients, including multi-disciplinary coordination as a case manager.
  • Identify and recommend referrals to other services as indicated, ensuring patient safety and well-being.
  • Establish relationships of trust and confidentiality with patients and caregivers, practicing infection control and meeting obligations for reimbursement.
  • Provide education and consultation to patients, caregivers, and the healthcare team, assessing learning needs and evaluating teaching outcomes.
  • Effectively identify and use appropriate educational tools and materials, planning educational programs to support team and departmental needs.
Requirements:
  • Bachelor's Degree
  • 1 year of relevant experience
  • Licensed Physical Therapist
  • CPR Certification (strongly preferred)
  • Home Health experience (strongly preferred)
  • Electronic Medical Records experience (strongly preferred)
  • Excellent communication skills
  • Advanced Certifications (strongly preferred)
