As an Independent Optometrist leasing space inside our Walmart stores, you will play a key role in delivering exceptional eye and vision care services to our customers. This opportunity allows you to establish your own practice, setting your own days of operation, hours, and patient schedule, while keeping 100% of exam and professional fees.

Key Responsibilities
• Establish and manage your own patient schedule
• Set your own fee schedule
• Control your days of practice and hours of operation
• Provide high-quality eye and vision care services to our customers

Requirements
• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9#
