SolutionHealth is seeking a dedicated and compassionate Geriatrician to join our team and provide part-time comprehensive medical care to our residents in a supportive and vibrant nursing home environment.

Key Responsibilities:

  • Conduct thorough initial and ongoing evaluations of residents' medical and psychosocial needs to ensure their well-being and quality of life.
  • Diagnose and treat a wide range of acute and chronic medical conditions common in older adults, utilizing evidence-based practices and staying up-to-date on the latest advances in geriatric medicine.
  • Develop and implement individualized care plans in collaboration with nursing staff, therapists, and other healthcare professionals to provide comprehensive care.
  • Manage residents' medications, ensuring safe and effective use, and provide education and support to family members and caregivers regarding their loved ones' care.
  • Participate in interdisciplinary care conferences and contribute to quality improvement initiatives to enhance resident-centered care.

Requirements:

  • Board-certified or board-eligible in Geriatric Medicine or Internal Medicine.
  • Valid medical license in the state of NH (or working towards).
  • Minimum of 2 years of experience working in a geriatric setting (e.g., nursing home, long-term care facility).
  • Excellent communication, collaboration, and interpersonal skills, with a compassionate and patient demeanor when interacting with older adults.
  • Strong commitment to resident-centered care and quality improvement, with proficiency in EPIC EMR.
