Senior Nutrition Therapist

Medical Nutrition Therapy Associates, LLC is a leading healthcare provider seeking a dedicated Senior Nutrition Therapist to join our team in a busy Long-Term Care (LTC) and Skilled Rehabilitation (SAR) facility.

As a Senior Nutrition Therapist, you will be responsible for delivering high-quality nutritional care to residents, ensuring their dietary needs are met while promoting overall well-being.

• Perform comprehensive nutritional assessments and ongoing evaluations of each resident's plan of care.
• Provide personalized nutrition counseling to residents and their families, addressing their unique needs and concerns.
• Participate in Interdisciplinary Care Conference (IDCC) meetings to ensure seamless collaboration with other healthcare professionals.

Key Requirements:

• Must be a Registered Dietitian with experience in LTC and Electronic Medical Record (EMR) systems.
• Strong organizational, communication, and interpersonal skills are essential for success in this role.
• Bonus points for experience in food-service management, as we strive for excellence in all aspects of patient care.
