Key Responsibilities:

  • Conduct thorough assessments of patients' daily living and work-related skills to develop personalized rehabilitation plans.
  • Administer and interpret diagnostic tests to assist physicians in evaluating patients' level of function.
  • Collaborate with healthcare team members to develop and revise treatment plans, ensuring a comprehensive approach to patient care.
  • Provide hands-on therapeutic treatment and instruction to patients, focusing on improving strength, coordination, range-of-motion, and function.
  • Review and expand rehabilitation plans to meet patients' behavioral goals and rehabilitation program guidelines.
  • Consult with vocational team members to develop vocationally oriented plans, promoting patients' return to work or daily activities.
  • Communicate patient status and progress to physicians, rehabilitation staff, and family members, ensuring a seamless care experience.
  • Participate in care coordination and discharge planning activities, ensuring a smooth transition for patients.
  • Design community reintegration activities to assist patients in physical reconditioning and psychological adjustment.
  • Recommend and design special adaptive equipment to improve patients' residence or working environments.
  • Train staff and family members to carry out rehabilitation programs, including dressing, feeding, grooming, and hygiene skills.
  • Supervise Occupational Therapy Assistant and Home Health Aide performance, ensuring high-quality care delivery.

Requirements:

  • Master's degree from an accredited Occupational Therapy Program.
  • Minimum of six months of occupational therapy experience, with a preference for Home Health experience.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • Valid driver's license, auto insurance, and reliable transportation.
