Nurse Practitioner or Physician Assistant, Acute Care Specialist

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Acute Care team at Capital Health Services. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and contributing to the development of our clinical programs.
Key Responsibilities- Conduct thorough patient assessments to identify priority care needs and develop individualized plans of care.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Perform procedures and implement care standards and protocols as needed.
- Develop and revise patient education programs to promote effective learning and outcomes.
- Participate in quality improvement initiatives and contribute to the development of clinical standards and guidelines.
- Lead and mentor team members to optimize teamwork and performance.
- Graduate of an accredited physician assistant program or a Master's degree in advanced clinical practice.
- One-year experience working in primary care management or acute care preferred.
- Current licensure as a Physician Assistant or Nurse Practitioner in New Jersey.
