Job Summary

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our Acute Care team at Capital Health Services. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and contributing to the development of our clinical programs.

Key Responsibilities
  • Conduct thorough patient assessments to identify priority care needs and develop individualized plans of care.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Perform procedures and implement care standards and protocols as needed.
  • Develop and revise patient education programs to promote effective learning and outcomes.
  • Participate in quality improvement initiatives and contribute to the development of clinical standards and guidelines.
  • Lead and mentor team members to optimize teamwork and performance.
Requirements
  • Graduate of an accredited physician assistant program or a Master's degree in advanced clinical practice.
  • One-year experience working in primary care management or acute care preferred.
  • Current licensure as a Physician Assistant or Nurse Practitioner in New Jersey.
