Patient Care Coordinator RN

We are seeking a highly skilled and dedicated Patient Care Coordinator RN to join our team at Sioux Land Community Health. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of high-quality patient care.

Key Responsibilities:
  • Triage and address patient phone calls and document appropriately.
  • Schedule appointments and make referrals.
  • Communicate and document provider's orders as necessary.
  • Access and update patient information in electronic medical records.
  • Refill patient medications per SCHC protocol/policy.
  • Prepare, update, and document in medical records.
  • Educate patients and distribute health information.
  • Address patient follow-up per provider's orders.
  • Use critical thinking skills to evaluate patient needs after emergency department or hospital dismissal.
Requirements:
  • Current RN licensure.
  • Current BCLS and Mandatory Reporter certification.
  • Solid computer skills.
  • Ability to work effectively in a team environment.
Preferred Qualifications:
  • Multi-Physician Family Practice experience.
  • Electronic Medical/Health Records (EMR) experience.
