Position Overview
MedStar Health is seeking a dedicated Specialty Clinical Pharmacist to enhance our Ambulatory Diabetes Clinical department. In this role, you will work closely with healthcare providers to promote the safe and effective utilization of specialty medications, ensuring seamless communication within the patient's healthcare team.
Key Responsibilities
As a Specialty Clinical Pharmacist, your responsibilities will include:
- Reviewing and assessing clinical guidelines to formulate and implement protocols.
- Conducting thorough medication reconciliation and developing comprehensive patient care plans.
- Providing patient consultations aimed at improving medication adherence and overall health outcomes.
- Documenting and reporting healthcare interventions and their outcomes.
- Participating in the maintenance of specialty pharmacy accreditation.
Qualifications
- Education: Doctoral degree (PharmD) from an accredited college of Pharmacy is required.
- Experience: A minimum of 3 years of direct patient care experience in a clinical setting, preferably within a specialty pharmacy, is required. Ambulatory care experience is preferred.
- Licenses and Certifications:
- Licensed Pharmacist in Maryland or the District of Columbia, as applicable.
- Eligibility for reciprocity to surrounding states is required.
- Completion of an ASHP accredited residency or equivalent advanced training is preferred.
- Board of Pharmacy Specialty certification (BPS) or other relevant certifications such as Certified Specialty Pharmacist (CSP) or AAHIVP are preferred.
Knowledge, Skills, and Abilities
The ideal candidate will possess:
- Strong verbal and written communication skills.
- Basic computer proficiency.
- Expertise in medication order verification, dosage regimen design, and drug therapy monitoring.
- Ability to create and maintain specialty medication patient care plans.
- Skills in providing education on specialty medications to patients and consultations with healthcare providers.
- Experience in obtaining patient and drug-related information for assessments and follow-ups.
- Capability to oversee specialty pharmacy activities within a clinical environment.
- Commitment to documenting services according to specialty pharmacy accreditation standards.
- Participation in collaborative practice agreements with prescribers to enhance patient care.
- Assistance with the implementation and management of new clinical programs requiring specialized monitoring.
- Coordination with clinical and administrative personnel to improve operational efficiency.
- Up-to-date knowledge of evidence-based clinical practice guidelines to align patient care plans with current research.
- Experience serving as a preceptor for student pharmacists, interns, and residents when applicable.
