**Job Summary**

Avante at Mt. Dora, Inc. is seeking a skilled Physical Therapy Assistant to join our team. As a Physical Therapy Assistant, you will work under the supervision of a Physical Therapist to provide high-quality Physical Therapy services to our patients.

**Responsibilities**

• Assist in assessment and treatment planning for patients

• Implement treatment plans as designed by the supervising Physical Therapist

• Document daily care as per established guidelines

• Communicate effectively with residents, family, and other team members

• Maintain accurate and up-to-date records of patient care

**Requirements**

• Associates Degree in Physical Therapy from an accredited program

• National Certification for P.T.A.

• Current state licensure or license eligible

• 0-3 years of long term care experience
