Your Role and Contributions:

The Palliative Care Coordinator plays a vital role in establishing and enhancing care as a clinical service line for patients and families facing acute medical challenges. This position involves conducting initial and ongoing clinical evaluations for patients with advanced or progressive health conditions referred for palliative care consultation. Key responsibilities include managing symptoms, aiding in decision-making—particularly in critical care environments and regarding non-beneficial treatments—collaborating with an interdisciplinary team to provide emotional, spiritual, and bereavement support, and coordinating care until the patient’s passing or discharge. The Coordinator adjusts care plans as necessary in collaboration with attending and consulting physicians, utilizing approved order sets, and serves as a resource for clinical inquiries. Participation in quality improvement initiatives and organization of interdisciplinary palliative care meetings is also essential.

The Value You Bring to the Team:

· Conducts initial palliative assessments with patients and families, including health history and psychosocial factors.

· Supports palliative care consultations and interventions for the early management of symptoms associated with life-limiting conditions.

· Collaborates with the patient’s physician and healthcare team to establish realistic care expectations aligned with the patient’s goals.

· Aids in the formulation and execution of the interdisciplinary treatment plan alongside the palliative care consultation team.

· Facilitates advance care planning for patients.
