We are seeking a skilled Healthcare Navigator to join our team. In this role, you will play a vital part in delivering exceptional patient care and collaborating with interdisciplinary teams to optimize outcomes.
Job SummaryThe Healthcare Navigator will provide expert navigation of the healthcare system, ensuring seamless transitions between care settings and facilitating communication among patients, families, nurses, physicians, and other healthcare professionals.
Key Responsibilities- Lead the coordination of patient care, monitoring the appropriateness and timeliness of care.
- Ensure the interdisciplinary care plan is consistent with the patient's clinical course, continuing care needs, and covered services.
- Discuss resource utilization, treatment plans, estimated length of stay, and discharge plans with physicians, focusing on complex patients and high-risk populations.
- Coordinate transfers to tertiary centers, including transfer of patient information for continuity of ongoing treatment and services.
- Provide oversight and collect data required for regulatory compliance and accreditation.
- Bachelor's Degree in Nursing (BSN) or equivalent experience.
- One year of professional nursing experience, with a focus on care management or case management.
- Registered Nurse (RN) licensure in the state of practice and Basic Life Support (BLS) certification.
