Key Responsibilities- Participate in decision-making, teaching, leadership, and quality improvement activities to enhance patient care outcomes and program operations.
- Identify and evaluate candidates for the Home Therapies Program, assessing patients' home environment and caretaker readiness.
- Perform assessments and identify barriers to home dialysis, providing education and support to patients and caretakers.
- Admit new patients to the Home Therapies Program, coordinating patient care and managing treatment plans.
- Provide initial and ongoing education to patients and families on home dialysis treatment and equipment operation.
- Ensure patient awareness of transplant and treatment modality options.
- Administer medications as prescribed, documenting medical justification and effectiveness.
- Initiate or assist with emergency response measures.
- Complete routine patient evaluations and ensure correct laboratory collection, processing, and shipping procedures.
- Collaborate with the Interdisciplinary Team on patient care plans and make referrals to Social Worker and Registered Dietitian as needed.
- Graduate of an accredited School of Nursing.
- Current appropriate state licensure.
- Current or successful completion of CPR BLS Certification.
- Must meet the practice requirements in the state in which employed.
- Qualities and traits: compassion, caring, support for colleagues, collaborative, reliable.
- Experience as a Registered Nurse (preferred but not required).
- Chronic/home hemodialysis experience (preferred but not required).
- Successfully pass the Ishihara Color Blind Test.
The position provides direct patient care that regularly involves heavy lifting, moving of patients, and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials.
