Job Summary

We are seeking a compassionate and skilled Spiritual Care Specialist to join our team at VNS Health. As a key member of our interdisciplinary team, you will provide direct spiritual care services and counseling to hospice patients and their family members.

Key Responsibilities
  • Implement the Spiritual Care program in accordance with CMS Conditions of Participation.
  • Assess the emotional, spiritual, and religious needs of patients and their family members.
  • Provide spiritual counseling and services, including crisis intervention, to meet the psycho-spiritual, religious, and existential needs of patients and their family members.
  • Develop and update the spiritual care component of the patient's plan of care based on mutually acceptable goals.
  • Collaborate with the IDG to ensure optimal quality care and document spiritual care assessments in the EMR.
  • Participate in weekly IDG team meetings and develop treatment plans as needed.
  • Act as a resource to VNS Health staff for clinical collaboration and interdisciplinary staff support.
Requirements
  • Bachelor's Degree in Humanities or related field required; Master's Degree in Divinity or Theology preferred.
  • Four units of Clinical Pastoral Education (CPE) required; Board Certified Chaplain preferred.
  • Minimum of one year of experience as a chaplain, preferably in a hospice setting.
  • Effective oral and interpersonal communication skills required.
  • Proficiency in personal computer operations, including MS Office applications required.
