We are seeking a highly skilled Nurse Practitioner to join our team in Johnson County, Texas. As a key member of our Advanced Illness Management team, you will play a critical role in providing high-quality, patient-centered care to individuals with complex medical needs.
Key Responsibilities- Assess and manage the care of patients with advanced illnesses, including those with cancer, heart failure, and chronic obstructive pulmonary disease (COPD).
- Develop and implement individualized treatment plans in collaboration with patients, families, and other healthcare professionals.
- Provide education and support to patients and their families to promote self-management and improve quality of life.
- Collaborate with the Palliative Medical Director and other healthcare professionals to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Master's degree in Nursing from an accredited institution.
- Current licensure as a Registered Nurse and Nurse Practitioner in the state of Texas.
- Minimum of two years of experience as a Nurse Practitioner in a palliative care or hospice setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Knowledge of palliative care principles, including symptom management and care coordination.
