Job Summary:

The Licensed Practical Nurse (LPN) is a vital member of our care team, responsible for delivering high-quality patient care in the comfort of their own homes. As a skilled home care professional, you will work closely with our clients, their families, and our interdisciplinary team to develop and implement personalized care plans that meet their unique needs and goals.

Key Responsibilities:
  1. Collaborate with our clients, their families, and our care team to assess and address their physical, emotional, and social needs.
  2. Develop and implement comprehensive care plans that promote independence, safety, and well-being.
  3. Provide skilled nursing services, including wound care, medication management, and vital sign monitoring.
  4. Conduct regular home visits to assess client progress, provide education, and make adjustments to care plans as needed.
  5. Communicate effectively with our clients, their families, and our care team to ensure seamless care coordination and timely interventions.
  6. Document client information, progress, and care plans in a timely and accurate manner.
Requirements:
  • Current and unencumbered LPN licensure in the state of practice.
  • Current CPR certification.
  • Infectious disease clearance for direct patient care.
  • Reliable transportation and a valid driver's license.
Work Environment:
  • Frequent travel required to client homes and other locations.
  • Ability to lift a minimum of 50 pounds and perform physical tasks as needed.
