About the Role

CAMBA is seeking a skilled Case Manager to join our team at the Park Slope Women's Shelter. As a Case Manager, you will play a critical role in supporting the health and well-being of our clients, providing them with the tools and resources they need to achieve stability and independence.

Key Responsibilities
  • Formulate and implement individualized plans to support clients in achieving their goals and securing permanent housing.
  • Conduct initial intake and assessment of clients' needs, as well as periodic re-assessments to ensure ongoing support.
  • Locate and refer clients to community resources, providing them with access to essential services and support.
  • Collaborate with clients to develop Independent Living Plans, setting clear goals and objectives for their future.
Requirements
  • Bachelor's degree in a related field, or equivalent experience.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment, with a high level of organization and attention to detail.
  • Bi-lingual preferred, but not required.
