We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Adoration Home Health and Hospice in Spring Hill, TN. This is a full-time opportunity that offers a flexible schedule, competitive pay, and a comprehensive benefits package.

About the Role

As an LPN with our company, you will provide high-quality, patient-centered care to individuals in their homes. Your primary responsibilities will include:

  • Providing 1:1 care for clients, following their individualized plans of care and promoting dignity and independence.
  • Monitoring vital statistics and reporting any abnormalities to the Case Manager and Physician.
  • Administering medications, feedings, oxygen, and ostomy care as needed, while accurately documenting results.
  • Completing routines established by other care professionals, such as PT, OT, and Speech Therapists.

You will also be responsible for ensuring infection control policies are maintained at all times, reporting possible communicable diseases or infections, and implementing infection control procedures. Additionally, you will accurately document nursing actions, communicate with family members, case managers, and physicians, and inform them of changes in client medical conditions and needs.

Requirements

To be successful in this role, you will need:

  • An LPN/LPT/LVN degree from an accredited college of nursing, with current unrestricted registration and licensure in the state of practice.
  • A valid driver's license, acceptable driving record, and proof of car insurance.
  • CPR professional certification.
  • Experience in administering medications.
  • 1+ years of nursing experience, preferably in home health care.
