Job Title: Physical Therapist (PT) SNF / LTC

Centra Healthcare Allied is seeking a skilled Physical Therapist to join their team for a contract assignment in Casper, Wyoming.

Job Summary:

The successful candidate will be responsible for providing high-quality physical therapy services to patients in a Skilled Nursing Facility (SNF) / Long Term Care (LTC) setting. The ideal candidate will have excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient care.

Responsibilities:
  • Assess patients' medical conditions, functional capabilities, limitations, and restrictions to develop an effective treatment plan.
  • Provide direct patient care, including physical therapy interventions, to help patients achieve their goals.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Document patient progress, outcomes, and any changes to the treatment plan.
  • Participate in ongoing education and professional development to stay current with best practices and industry developments.
Requirements:
  • Graduation from an accredited physical therapy program.
  • Current state license as a Registered Physical Therapist (PT) or actively pursuing licensure.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
