Housing Case Manager

Job Summary:
The Housing Case Manager will provide case management and supportive services to clients, promoting self-sufficiency and stability. Key responsibilities include developing and implementing individualized service plans, advocating for clients' needs, and maintaining accurate records.
Key Responsibilities:
- Provide case management services to clients, including assessment, goal setting, and progress monitoring.
- Advocate for clients' needs, ensuring access to necessary resources and benefits.
- Maintain up-to-date and accurate client records, adhering to agency policies and external requirements.
- Develop and facilitate scheduled monthly support group meetings.
- Prepare routine and ad-hoc reports as required.
- Know and comply with relevant laws and regulations.
- Participate in community outreach efforts.
Requirements:
- Bachelor's Degree in Social Work or related field.
- At least 1-2 years of experience in case management or a related field.
- Possess excellent communication and interpersonal skills.
- Ability to work independently and collaboratively as part of a team.
- Familiarity with community resources and social services.
Work Environment:
This is a full-time position requiring occasional evening and weekend work. The ideal candidate will have reliable transportation and be willing to travel within the city.
Compensation and Benefits:
We offer a competitive salary of $55,000 - $65,000 annually, depending on experience, plus comprehensive benefits, including medical, dental, and vision insurance, retirement plan, and paid time off.
